Deepak Dhuna is Senior Cognitive Behavioural Psychotherapist with the Primary Care Psychological Therapies Team and will be joining this locality 1 day a week thought the Additional Roles Reimbursement Scheme initiative. He has a special interest in parity of care and will be focusing on health inequalities within the locality for ethically and racially minoritized communities. He looks forward to working with our patients and the local community to help make Mental Health and Wellbeing Support easier to access.

Postural Stability

What is the Postural Stability Programme- (Falls Prevention)?

Active Leeds offer a programme of activities for people at the risk of falls. These ‘Strength and Balance’ sessions are designed to improve strength, balance and co-ordination for people with low mobility. The sessions involve a mixture of chair based exercises and standing strength and balance exercises.

Eligibility Criteria:

  • Age 60+
  • Able to walk (aided or unaided) and transfer from seated
  • Able to follow instructions independently
  • Mild or moderately frailAt risk of falling / have had a fall(s)
  • No contraindications to health (e.g. unstable heart conditions)

The programme includes;

  • Group activities in the community.
  • Using the Keeping Well at Home Resources.
  • Support via phone or online.
  • There are a limited amount of loan “digital tablets” that people can borrow. This means people that are digitally excused can access online resources to help them keep active

Additional to our land based programme we also run a 6 week hydrotherapy pool programme for individuals who would feel more confident in a warm pool. These sessions are for people who are eligible for the Strength and Balance programme. A referral is required to access the programme, please ask your health and wellbeing coach for more details
Aqua- Mobility

We run Aqua – Mobility sessions which are open to anyone who feels they would benefit from low intensity movement in water. Helping improve their balance and strength. Sessions are delivered in the main pool and promotes gentle exercise in water. There is no loud music played in this class and no referral required, it’s open to all.

Mental Health Link Workers

Living Well Mental Health Practitioners do mental health support differently, with a focus on your skills, aspirations and experiences.

We support people who experience mental health difficulties to work towards recovery, stay well, make their own choices & take part in everyday life.

They work closely with our GPs and can talk to you about the wide range of treatments that can help you. This may include medication options, therapy or support with relationships, housing, debt or employment.

Whether you’re experiencing new or existing mental health problems, our Mental Health Practitioners are here to offer support that stops difficult times from becoming worse.

The ARRS psychology roles are newly established roles based in primary care. They have been created to support the development of specialist mental health input directly into GP teams and PCNs. 

It is hoped that offering psychological thinking and psychological perspectives will be of benefit to both staff and the practice population alike.

This input consists of direct clinical work with practice patients and contributing to the formulation of a patient’s presenting difficulties. I can also offer case consultation with staff regarding individuals seen by the practices.

Pharmacists

Clinical Pharmacists

PCN Clinical pharmacists are experts in medicines, the effects they have on the body and how and when they are used across many medical conditions.

The clinical pharmacists in the PCN team work on specific projects with the aim to improve health inequality and health outcomes across the population of Beeston.  We are also a key part of the multi-disciplinary team working across our care homes and those patients in the community that are severely frail.

The clinical pharmacists review medications, assess and monitor long term conditions such as heart failure and high blood pressure.  We also undertake in-depth medication reviews with patients that take more than 10 medicines and have specific conditions and review all the medicines taken by our care residents annually. We offer face to face, telephone and in home consultations to ensure fair access to all.  These consultations may also include a physical examination such as blood pressure measurement, heart rate, temperature, respiratory examination etc.

Physiotherapists

A first contact practitioner (FCP) is an experienced physiotherapist who has the advanced skills necessary to assess, diagnose and recommend appropriate treatment or referral for musculoskeletal (MSK) problems on a patient’s first contact with healthcare services e.g. when they visit their GP surgery.

What is First Contact Physiotherapy (FCP)?

  • Physiotherapists working in Primary Care
  • First point of contact for patients with musculoskeletal pain (muscles and joints)
  • Alternative to the GP not addition or for second opinion.
  • Aim to provide early access to specialist knowledge and assessment.
  • Majority of aches and pains resolve with the right advice
  • Usually one appointment
  • It is NOT a course of Physiotherapy

PCN Registered Nurse Associates

  • We have a newly created Registered Nurse Associate role within the PCN. They do variety of clinics such as smears, immunisations, vaccinations, dressings, phlebotomy. They also do frailty assessments.
Social Prescribing

Social Prescribing

Social prescribing is a relatively new approach to address ‘what really matters to you’. 

A social prescriber (also known as a link worker) gives people extra time and support to make decisions based on their health. Social prescribers understand that some issues cannot be treated by medication, and those issues usually arise from things that matter to you, such as housing problems, loneliness, anxiety and finances and other things that really affect you. Social prescribers both understand that you may wish to have more to discuss with your GP after your appointment, and social prescribers are here to support you on matters that cannot be fixed by one appointment alone. 

We work closely with our local partners who are experts in their fields and that can work with you and provide the time to support you, whether that be offering details of your local clubs and societies, pointing you in the right direction for housing assistance and support, and many other areas which really matter to you. There is emerging evidence to suggest that this approach to personalised care is much more beneficial to patients, and assists patients in thriving further onwards to their health related goals and aspirations. 

The PCN has its very own social prescribers that have their own specialities in certain areas, such as championing learning disability rights and access to appointments, advising on the importance of immunisations, vaccinations and health screening, and supporting patients who need additional help to age well and healthy.
