About Us

What is a Primary Care Network?

A Primary Care Network (PCN) is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.

Who are Beeston, Middleton and Hunslet PCN?

Our PCN is unique, at the fact that we can work across 2 PCNs.

Middleton & Hunslet PCN is made up of 4 GP practices, with a patient population of around 33,000.

Beeston PCN is made up of 4 GP practices, with a patient population of around 47,000.

Dr Kashif Sarwar

Clinical Director,
Beeston PCN

I am a GP Partner at Leeds City Medical Practice and the Clinical Director of Beeston Primary Care Network (PCN). My driving factor to become involved in my PCN role was to help support the local Practices and PCN staff to improve the services we provide to patients. This involves working in partnership with community and hospital services, social care, and other providers of health care services.


The PCN team now has a multitude of professionals working within Practices providing direct care to patients.The Beeston PCN team are involved in several projects involving a variety of different professionals to further improve quality of care to patients, such as collaborating Doctors, Nurses, Pharmacists, care coordinators and care home staff to provide excellent care to our care home residents. Another project has been the Heart Failure Pilot where the PCN team have worked closely with the local Cardiologists and Heart Failure community team to improve the care to those suffering from Heart Failure.


One of the biggest challenges that the PCN, and the NHS, has faced in recent times has been the Covid-19 pandemic, and the difficulties this has brought to NHS teams to deliver safe, good quality care to our patients. However, the pandemic has also brought many positives, such as allowing Practices to utilise technology better to consult with patients. In addition, I feel that the excellent roll-out and contribution to the Covid-19 vaccination programme by the Beeston PCN and Practice teams has been a great success story in that it not only helped protect our patients and colleagues from Covid-19, but it also allowed the opportunity for the Beeston Practices and wider PCN team to work very closely together and build our relationships with one another.

In 2022, I transitioned to the NHS after a career in the Private Financial Sector. My role as a Project Manager at Leeds Teaching Hospitals was centred on reducing the Length of Stay for patients during hospital admissions.


In my role as Business Manager, I concentrate on leading the team, driving change, and achieving results that enhance support for our patients and practices. This includes fulfilling our contractual commitments, securing a robust financial status, and initiating new services to promote the health of our patients throughout the South and East of Leeds.


I am passionate about delivering better health care for all, to support our Primary Care function in reducing wait times as well as ensuring our patients are seen by the most appropriate clinician the first time. I am proud to be a part of such a strong team and excited to where we are able to take this in the near future.

Hi, my name is Steph, I am the Nurse Manager for Leeds City Medical Practice and the Clinical Nurse Lead for Beeston PCN and support Middleton & Hunslet PCN.


I have been working at Leeds City since July 2013; understanding, developing, implementing, and delivering services for our practice population. Manage, support, and help develop an effective, cohesive, and quality nursing team.


In 2017, I was appointed Locality Practice Nurse Lead for the Leeds 10 & 11, to help collaborating working for the GP Practices within Leeds 10 &11. During this time, we developed and established the existing Beeston & Middleton/ Hunslet PCN Dressing Clinic which is continuing to improve, develop and provide an efficient, reliable, and quality service for both PCN’s.


June 2019, the PCN’s were established, and I took on the role of Clinical Lead Nurse for Beeston PCN. Through the pandemic, the Beeston PCN Executive Team planned, developed, and implemented a highly successful collaborative Beeston PCN Covid Vaccination clinic, sourcing staff from admin to clinical.


I have helped to develop a Patient Ambassador programme for Beeston and Middleton/Hunslet PCN. Recruited and established training and developing for Hybrid HCA , Nurse Associates and working with the Frailty Lead to create a quality and innovative service that will benefit and target our PCN population.

Bev McLean

PCN Manager,

I have worked in Primary care now for nearly 18 years. My career started with the Halifax Bank in Glasgow where I was the first internal Management Trainee in Scotland. I spent nearly 16 years with them working in various managerial roles moving down to Yorkshire in 1993 to work in the Treasury Department in the head office. I left there taking Voluntary Redundancy as a Senior Manager when the department relocated to London.


I then took a year out to study a Masters in Personnel and Development at Leeds Metropolitan University before taking up the challenging role of Practice Manager at a GP practice in Calderdale. During my time in Calderdale I was involved in our local PCN and I project managed a full relocation of the practice into new purpose built premises. Laterally I also managed a merger with another smaller local practice. 


Early in 2020 I moved over to Leeds City to take up the post of Business Manager. Leeds City offered me a new challenge – a much bigger practice with a totally different demographic and structure. The team at Leeds City are very welcoming and supportive, I really enjoy working here as part of the team. The last two years have not been how I envisaged with all the challenges COVID brought with it, but things are easing now and I hope this year I will be able to focus on developing the practice as I had originally planned.


I took up the role of Beeston PCN Business Manager a few months after joining Leeds City, as I wanted a better insight into how local practices work and wanted to be able to help in the development and delivery of all the new services and opportunities that now come through to the network. The two PCNs are well established, with two great Clinical Directors who work well together and they get things done, it is a real privilege for me to part of it.

In our role with the Leeds GP Confederation, we work alongside the Primary Care Network to provide resource for individual practices as well as supporting the collective development of the Networks.

Our primary goal is to support resilience and collaboration and day to day activity is determined by the needs of the PCN at any given point as they deliver care to their patients. We support the PCN and their member practices to improve the health outcomes for local populations whilst maintaining a focus on addressing health inequalities and ensuring those living in deprivation or who are vulnerable have access to the care they need.

Since February 2024, I have been employed as the Patient, Systems & Admin Co-ordinator for Beeston, Middleton and Hunslet Primary Care Network. I previously worked as a supervisor for a busy Medical Practice in the North Leeds area for over 7 years, which is where I gained my knowledge and passion for the NHS.

I was keen to make the transition from working within a specific GP surgery to working for the PCN when I learnt about the amazing work that they do to support the surgeries in their area and how they are able to offer a wide range of accessible services to their patients.

I am excited to support the PCN and grow within my new role as a Patient, Systems & Admin Co-ordinator by helping to promote digitalisation and supporting the clinical and Executive teams with all things administrative.

Care Home Team Lead, Middleton & Hunslet PCN

Hi, my name is Louisa. I am an advanced nurse practitioner for Lingwell Croft Surgery and a Clinical Lead for Middleton and Hunslet PCN. I have been a nurse for over 35 years, working in the community and Primary Care for the last 25 years.

My areas of interest and expertise are with older adults with a particular interest in frailty.

During my time with the PCN I have worked on the Enhanced Health in Care Homes Scheme across both Beeston and Middleton/Hunslet PCNs. This began with recruiting for 2 Care Home Coordinators who have since worked on aligning one GP practice to a care home in order to build a more comprehensive team and provide continuity of care. It has also involved establishing weekly multi-disciplinary meetings for each care home, in order to improve quality of care for the residents.

I have also been involved with the Enhanced Frailty Scheme across the PCNs which aims to provide proactive, coordinated care to our population with severe frailty.

Recently I have started as Clinical Lead for Middleton and Hunslet and am excited to be broadening my areas of knowledge for this post as well as supporting the growing PCN team.

Why I chose to work for this PCN? – Previously working in a GP practice within this PCN, I saw the incredible work of the PCN and wanted to be part of that!

Hi, my name is Bev, Senior Pharmacist for Beeston PCN. I attained a first class honours MPharm at University of Reading in 2015, prior to that I was a pharmacy technician for 16 years.  The majority of my career has been in secondary care and the switch to primary care has been challenging but very rewarding. Using the backdrop of the Network DES and implementation of PCN’s to try and improve health outcomes for patients in an area that is in the top 1% of socio-economic deprivation in England.  I feel passionate about doing the best for those communities by working in innovative ways to ensure we access the hardest to reach. 

Mehmoona Tabassum

Hi my name is Mehmoona Tabassum, I am currently a Clinical PCN pharmacist in Beeston, Leeds. I am an independent prescriber and have a special interest in hypertension. I thoroughly enjoy working as part of a diverse multidisciplinary team within general practice. My role has allowed me to provide one to one advice and guidance to patients with uncontrolled hypertension via the home BP monitoring project. As a result many patients have benefited in a reduction in BP and overall physical well-being.

I am the Pharmacy Technician for Beeston PCN. Previous to starting my new role with the PCN I hadn’t had much experience working in/for GP practices so this was all quite new to me. I have learnt so much since starting my new role and have become a lot more confident and competent each day. I thoroughly enjoy my role here and have had and still have the best support from my team. I have recently set up CPCS for my PCN and have had fantastic feedback from the local community pharmacies and the practices; this has been a good project to work on to create good working relationships with my local community pharmacies and practice staff. A new pharmacy technician has just recently joined our team for Beeston PCN. Our plan is to start having our own face-to-face clinics for a scheme we are currently working on for home BP monitoring for our patients here in Beeston. This will be good for us to communicate and get to know our patients and also enhance our clinical skills. I enjoy engaging with our patients and feel good knowing I have made a positive difference to them. Working for Beeston PCN has been a fantastic experience for me, as I continue to learn and grow within my role every day and most importantly working to help our patients here in Beeston.

Hi! I’m Alice, one of the new PCN pharmacists in the team. I graduated from University of Huddersfield in 2016 with a Master of Pharmacy (MPharm). I initially started in community pharmacy, then moved into hospital outpatient pharmacy and then finally made the big jump to the scary primary care world! I also pursued a Clinical Diploma (PgDip) from the University of Manchester in 2021. 

I have always wanted to go down a more clinical route in my career, so I figured primary care was the best place for this – I am currently on the primary care pathway on track to become an Independent Prescriber. I have a special interest in women’s health and hope to further develop my knowledge in this area. I chose Beeston PCN as it is full of friendly and incredibly skilled colleagues which makes working together easy and beneficial to achieving the best patient outcomes. Within the PCN the work is varied and there are many routes you can choose for career development.

My career aim has always been to make a difference to patients’ lives. Here in the Beeston community, I aim to provide adaptive and accessible care to suit our patients’ varied needs.

Hello. My name is Mel, and I’m one of the new pharmacists to join the Beeston PCN team. I graduated from King’s College London with a Master of Pharmacy (MPharm) in 2016. I returned to Yorkshire in 2017 and since then I have worked mainly in community pharmacy, so this is a completely new challenge for me!

Community pharmacy taught me lots of skills and I thoroughly enjoyed the interaction with patients, but I wanted to do more and have more involvement with their care. I chose Beeston PCN as it is the perfect mix of skillset, where we can all work together to achieve better outcomes for patients who are typically underrepresented. Through my role in medicines optimisation and achieving specific targets to reduce health inequalities, I’m excited to see what I can do to support the team in ensuring the local community we serve can get the best from their NHS.

My name is Sarah Kent and I am a Clinical Psychologist working within Beeston PCN. I work as part of the primary care mental health team, supporting my colleagues within the team, as well as offering some one-to-one assessments and individual and group interventions.

It is a really new idea to have psychologists working within PCNs, and I am really excited about how this will develop. Hopefully there will be benefits for both staff and patients who are a part of the PCN.

My names is Eilish and I am the Frailty Advanced Clinical Practitioner (ACP) for Beeston PCN.

I was excited to work for the Beeston PCN and help to create a new service to support patients living at home with sever frailty. Having worked for the neighbourhood team and the acute trust I am looking forward to how we can bring these services together to support patients at home. This is an exciting project and I am looking forward to how we can shape this in order to best support our patients.

Working with the multidisciplinary team we have discussed patients with pharmacist, social prescribers, ACPs and nurse associates to make cares plan to best support patient’s health and social care needs. During this we were able to support an elderly lady living with sever frailty to leave the house for the first time since COVID and attend a ballroom dancing class.

I qualified from York St John University with a BSc (Hons) Occupational Therapy degree in 2009.

Over the last 13 years, I have worked within adult mental health services and more predominantly across acute hospital and community rehabilitation services in Greater Manchester.

This has included working within many specialisms such as acute medicine, surgery, orthopaedics, stroke, intermediate care and early supported discharge/community neuro rehabilitation. Over the last 8 years, I have worked at a senior level across acute stroke and stroke/neuro rehabilitation which has enabled me to build a solid knowledge and skill base in the assessment and treatment of people with complex diagnosis, impacting on their ability to manage everyday activities.

Whilst I have enjoyed working in this area, I’m really excited to be taking on this new role within the Beeston, Hunslet and Middleton PCN. I’m keen to continue to promote a culture of personalised care and health equity within the team, to deliver appropriate occupational therapy assessment and treatment and to build a stronger network of links with other services to enable our community to access the right team/professional at the right time.

Hi, I am Hayley and I qualified as an Occupational Therapist in 2011. I started my career working primarily in an Older Adults Community Mental Health Team with a focus on dementia. Specialising in this area within a community setting enabled me to develop skills in the assessment and treatment of older adults with complex physical and mental health needs.

Although I enjoyed working within this specialism, I am really looking forward to this next journey in my career working in the Beeston, Middleton and Hunslet PCN.

I am passionate in providing and promoting person centred care and I am keen to consolidate my skills in the primary care setting. I feel that I have joined the PCN at an exciting time. Whereby we are developing and expanding the OT offer to practices, whilst continuing to provide strategic leadership and developments to the frailty team.

I’m Sara I’ve worked for the PCN for a year now.  I can honestly say it’s the best thing I’ve ever did for my career and own personal development. I have never worked for the NHS before and have gained much knowledge around Primary Care services and am excited about my journey and personal development working within in the Beeston and Middleton PCN family. The future is bright!

I chose this PCN because I was local resident and use to access these services myself and understand how vital and important the services that are being delivered. I want to now give back to a community that helped me be the strong caring and resilient person I am today.   

I am a Patient Ambassador and work within social prescribing and triage and signpost patients to holistic and continuing support within in the wider third sector. I have 5 years of experience in the third sector supporting older people helping them live later life well.  

I enjoy empowering people to make positive changes in their life chooses.

My name is Alma and I have been part of the PCN team since September 2023. As one of the Social Prescriber Link Workers, my focus is on patients with learning disabilities.

Day to day social prescribing for me may include supporting a patient with LD/SMI to an appointment, referring on to relevant services, ensuring patients are connected with wider community groups and services, etc.

Part of my role is to ensure all services can be fully accessed for people with LD and SMI, exploring any potential barriers in place preventing patients accessing all health and wellbeing services available.

As from April 2022, I have been employed as a Social Prescriber Link Worker with the responsibility for promoting and increasing the uptake of Childhood Immunisations and Cancer Screenings e.g. Bowel, Breast and Cervical and also promoting Prostate cancer awareness. I support reduction of health inequalities across our community by linking in with local schools, nurseries and charitable organisations in Beeston. As the Beeston Primary Care Network’s (PCN) Digital Lead, I am familiar with the Digital Health Hubs across Beeston, Middleton & Hunslet.

I joined Lingwell Croft Surgery in Middleton in 2018, becoming Clinical Director for Middleton and Hunslet PCN shortly after arriving. I have always been passionate about the galvanisation of general practice and tackling health inequalities within the most deprived areas. Becoming Middleton & Hunslet’s Clinical Director was therefore the perfect opportunity to get involved with shaping the evolution of care within the PCN’s local community, not only to support local practices better but to develop an integrated multidisciplinary approach to care.

The COVID-19 pandemic presented a significant challenge to the Middleton & Hunslet PCN and had a substantial impact on the ability to develop services during this time. One positive outcome however, was the banding together of GP surgeries to support one another and overcome the challenges together. This created a closer and more collaborative working relationship moving forwards.

We have now had the opportunity to develop services addressing inequalities in the area. We have seen success in offering support to care homes, providing personalised care plans to those living with frailty, improving cancer screening uptake, and setting up local enhanced services for patients with diabetes, COPD, heart failure and mental health problems.

The future in bright! As the PCN grows, I believe the local community will notice a change in how care is delivered, and those in greatest need will be able to get the support they need.

My name is Sophie and I am the Managing Partner at Lingwell Croft Surgery and the PCN Manager for Middleton and Hunslet PCN.

I have worked in Primary care for 13 years, initially at a GP practice in Barnsley before moving to Lingwell Croft Surgery in Middleton, Leeds.

My career started working for a large national retail company where I was based in Manchester, I worked for them for 12 years around Cheshire and had various project management roles including national lead on customer service training.

I then moved to Sheffield in 2003 where I worked as a Manager at the ‘Places for People housing association’. I was the central point of contact and advice for a diverse and complex group of companies within the organisation, such as the housing development team, refugee schemes, homeless schemes, sheltered housing schemes and neighbourhood regeneration schemes. I was also lead for the tenant’s association who met monthly. I studied for a PGCert in housing and politics whilst I was there which really helped me understand issues around social history and housing and how it can affect people’s physical and mental health.

I have been involved in volunteer work where I live for 18 years, helping with gardening, organising community events and cultural twinning exchanges with people from overseas. Our biggest achievement was helping raise the funds and building a community centre with a dance studio and fully fitted kitchen, changing rooms and disabled access.

I joined the PCN when it formed as I had previous experience of setting up and running the first home visiting Phlebotomy service in Leeds for our local practices, which was very successful and has been rolled out city wide since then. I believe that we can provide a much better service to patients if as a group of practices, we work together and share resources and ideas. We can shape and commission the specific services that we know is needed for the patients in our areas.

We work closely with Beeston PCN who are our neighbours; and together we are a great team with a good working relationship. We really couldn’t have managed the covid vaccination service successfully if we hadn’t all worked together.

I was keen to make the transition from working within a specific GP surgery to working for the PCN when I learnt about the amazing work that they do to support the surgeries in their area and how they are able to offer a wide range of accessible services to their patients.

I am excited to support the PCN and grow within my new role as a Patient, Systems & Admin Co-ordinator by helping to promote digitalisation and supporting the clinical and executive teams with all things administrative.

Hi, my name is Louisa. I am an advanced nurse practitioner for Lingwell Croft Surgery and a Clinical Lead for Middleton and Hunslet PCN. I have been a nurse for over 35 years, working in the community and Primary Care for the last 25 years.


My areas of interest and expertise are with older adults with a particular interest in frailty.


During my time with the PCN I have worked on the Enhanced Health in Care Homes Scheme across both Beeston and Middleton/Hunslet PCNs. This began with recruiting for 2 Care Home Coordinators who have since worked on aligning one GP practice to a care home in order to build a more comprehensive team and provide continuity of care. It has also involved establishing weekly multi-disciplinary meetings for each care home, in order to improve quality of care for the residents.


I have also been involved with the Enhanced Frailty Scheme across the PCNs which aims to provide proactive, coordinated care to our population with severe frailty.


I have started as Clinical Lead for Middleton and Hunslet and am excited to be broadening my areas of knowledge for this post as well as supporting the growing PCN team.

Why you chose to work for this PCN? It has a great team and they do fantastic work supporting care homes & GP practices, the benefits of working for the PCN was attractive and I wanted to be a part of it.

I chose to work at Middleton and Hunslet PCN as it is has a fantastic team with a diverse skill mix of professionals who collaborate well together. The clinical pharmacy team are a key part of the multi-disciplinary team working across our care homes and those patients in the community that are severely frail. We also work on specific projects with the aim to improve health inequality and health outcomes across the population Middleton and Hunslet. 

 The clinical pharmacists have successfully run medication review clinics for long term conditions such as heart failure, high blood pressure and chronic pain.  We have also undertaken in-depth medication reviews with patients that take more than 10 medicines and have specific conditions. We offer face to face, telephone and in home consultations to ensure fair access to all.

I chose to work for a PCN because I wanted the experience of working with multidisciplinary health teams and to use more of my clinical skills.  My background is community pharmacy and health education, and I felt that my skills would be a good fit working in a PCN, with a wider scope for developing these skills, as well as learning new skills. I love liaising with patients and problem solving and this role has both of these in abundance!

I’ve not been in post for long.  I did identify someone who was having issues with resistant hypertension, only to find that she was also on medication to treat hypotension.  I think the patient discussions that I feel are the most successful are patients who intentionally are non-adherent with medication.  Building the rapport and trust for the patient to be honest about their views of the medication, then being able to agree a solution or a compromise: this always feel like a success to me as I know that a patient has made an informed choice about their own health and that I have been able to support them with this.

I work as a Clinical Psychologist within the Middleton/Hunslet PCN. In my current role I provide 1:1 psychological interventions and through indirect work and joint formulation sessions I offer consultation to members of the multidisciplinary team. I recognise the importance in being able to link client difficulties with psychological models such as attachment, learning and behavioural theories. I have gained specific experience in delivering cognitive analytic therapy, CBT and psychoanalytic therapy. I attempt to use such approaches discerningly and flexibly with clients from varying backgrounds and with a range of psychological difficulties in order to bring about positive change. Outside of my NHS employment I have worked privately providing mindfulness and motivational interviewing training to members of staff working both within physical and mental healthcare NHS settings. I enjoy keeping fit and active and I am also a keen musician and songwriter. I have an interest in clinical decision making and have presented the main findings from my doctoral thesis at the BABCP Conference in Bath. My thesis looked to design and test a trial-based methodology to assess the influence of bias on decisions regarding treatment allocation and progression. I am hopeful that my study will eventually be peer reviewed and published and I am currently in the process of working towards this. I am committed to maintaining and improving the quality of patient care within mental health and recognise the importance of bringing psychological intervention and formulation closer to communities, either through GP surgeries or in alternative locations embedded within primary care.

Since qualifying in 2018 in Budapest Hungary, I have built up a vast array of experience working at Neurological ward and ICU department treating many different conditions and being in charge of my own patient and work load and patient progression. I have also worked in private clinics treating many different conditions and have a good knowledge and understanding of a variety of musculoskeletal conditions. In October 2022 I moved to Pure Physiotherapy working as a First Contact Practitioner (FCP) for NHS primary care networks, this has brought more strength to my clinical reasoning, and providing me with great knowledge and experience to help all of my patient.

From working in private physiotherapy clinic in the UK, I have only had the opportunity in treating MSK conditions. The reason I moved to an NHS PCN was to help me to increase my skills, work with a large MTD and become more advance with my diagnosis and treatment plan, adding another step in my ladder towards Advance Physiotherapy Practitioner.

Since in post I have been able to screen individuals for medical conditions that may present as MSK problems and then refer onwards for the appropriate investigations of treatments. I was able to positively asses and help the patients that have walked into the room and provided them with peace of mind a good Laugh and home exercises or the correct pathway for referral.

I qualified from York St John University with a BSc (Hons) Occupational Therapy degree in 2009.

Over the last 13 years, I have worked within adult mental health services and more predominantly across acute hospital and community rehabilitation services in Greater Manchester.

This has included working within many specialisms such as acute medicine, surgery, orthopaedics, stroke, intermediate care and early supported discharge/community neuro rehabilitation. Over the last 8 years, I have worked at a senior level across acute stroke and stroke/neuro rehabilitation which has enabled me to build a solid knowledge and skill base in the assessment and treatment of people with complex diagnosis, impacting on their ability to manage everyday activities.

Whilst I have enjoyed working in this area, I’m really excited to be taking on this new role within the Beeston, Hunslet and Middleton PCN. I’m keen to continue to promote a culture of personalised care and health equity within the team, to deliver appropriate occupational therapy assessment and treatment and to build a stronger network of links with other services to enable our community to access the right team/professional at the right time.

I’m Sara I’ve worked for the PCN for a year now.  I can honestly say it’s the best thing I’ve ever did for my career and own personal development. I have never worked for the NHS before and have gained much knowledge around Primary Care services and am excited about my journey and personal development working within in the Beeston and Middleton PCN family. The future is bright!

I chose this PCN because I was local resident and use to access these services myself and understand how vital and important the services that are being delivered. I want to now give back to a community that helped me be the strong caring and resilient person I am today.   

I am a Patient Ambassador and work within social prescribing and triage and signpost patients to holistic and continuing support within in the wider third sector. I have 5 years of experience in the third sector supporting older people helping them live later life well.

My name is Alma and I have been part of the PCN team since September 2023. As one of the Social Prescriber Link Workers, my focus is on patients with learning disabilities.


Day to day social prescribing for me may include supporting a patient with LD/SMI to an appointment, referring on to relevant services, ensuring patients are connected with wider community groups and services, etc.

MDT Case Study

Below you can find a case study that highlights the efficacy and coordination of our team at Beeston PCN.

“I received an email from the reception practice staff at LCMP on a Friday afternoon informing me of a patient, male 70 years old that had enquired within practice that he was not happy with the current service he had had with his current surgery (the patient in regard to previous surgery was not part of the PCN, it was one outside of our remit). He mentioned he was suffering with depression and anxiety and had several health concerns, and that he hadn’t seen his GP or received a call in 3 months. As a result he was incredibly worried about his health.

I phoned the patient and managed to register him to the practice online, and after having a quick chat with patient I was able to ascertain needs such as loneliness and his concerns regarding his healthcare. After this conversation I put him in touch with the outreach worker at a community group which offered support and digital inclusion and social activities, then emailed back the practice the details of the conversation we had regarding his health.

I checked the patients details on the following Monday morning and the patient had been registered and already had an appointment booked in for a weeks time. After looking at the patients details he attended his appointment and has a follow up for 3 weeks time.”
